Respect for autonomy has become a fundamental principle in human research ethics. Nonetheless, this principle and the associated process of obtaining informed consent do have limitations. This can lead to some groups, many of them vulnerable, being left understudied. This book considers these limitations and contributes through legal and philosophical analyses to the search for viable approaches to human research ethics. It explores the limitations of respect for autonomy and informed consent both in law and through the examination of cases (...) where autonomy is lacking (infants), diminished (addicts), and compromised (low socio-economic status). It examines alternative and complementary concepts to overcome the limits of respect for autonomy, including beneficence, dignity, virtue, solidarity, non-exploitation, vulnerability and self-ownership. American society has a history of turning to physicians during times of extreme need, from plagues in the past to recent outbreaks of communicable diseases. This public instinct comes from a deep seated trust in physician duty that has been earned over the centuries through dedicated and selfless care, often in the face of personal risks. As dangers facing our communities include terroristic events physicians must be adequately prepared to respond, both medically and ethically. While the ethical principles that govern (...) physician behavior—beneficence, nonmaleficence, autonomy, and social justice—are unchanging, fundamental doctrines must change with the new risks inherent to terroristic events. Responding to mass casualty disasters caused by terrorists, natural calamities, and combat continue to be challenging frontiers in medicine. Preparing physicians to deal with the consequences of a terroristic disease must include understanding the ethical challenges that can occur. (shrink)

Drawing the line on physician assistance in physician-assisted death (PAD) continues to be a contentious issue in many legal jurisdictions across the USA, Canada and Europe. PAD is a medical practice that occurs when physicians either prescribe or administer lethal medication to their patients. As more legal jurisdictions establish PAD for at least some class of patients, the question of the proper scope of this practice has become pressing. This paper presents an argument for restricting PAD to the terminally ill (...) that can be accepted by defenders as well as critics of PAD for the terminally ill. The argument appeals to fairness-based paternalism and the social meaning of medical practice. These two considerations interact in various ways, as the paper explains. The right way to think about the social meaning of medical practice bears on fair paternalism as it relates to PAD and vice versa. The paper contends that these considerations have substantial force when directed against proposals to extend PAD to non-terminally ill patients, but considerably less force when directed against PAD for the terminally ill. The paper pays special attention to the case of non-terminally ill patients who suffer from treatment-resistant depression, as these patients present a potentially strong case for extending PAD beyond the terminally ill. BACKGROUND -/- Understanding, modelling and influencing the transition between different states of cells, be it reprogramming of somatic cells to pluripotency or trans-differentiation between cells, is a hot topic in current biomedical and cell-biological research. Nevertheless, the large body of published knowledge in this area is underused, as most results are only represented in natural language, impeding their finding, comparison, aggregation, and usage. Scientific understanding of the complex molecular mechanisms underlying cell transitions could be improved by making essential pieces of (...) knowledge available in a formal (and thus computable) manner. -/- RESULTS -/- We describe the outline of two ontologies for cell phenotypes and for cellular mechanisms which together enable the representation of data curated from the literature or obtained by bioinformatics analyses and thus for building a knowledge base on mechanisms involved in cellular reprogramming. In particular, we discuss how comprehensive ontologies of cell phenotypes and of changes in mechanisms can be designed using the entity-quality (EQ) model. -/- CONCLUSIONS -/- We show that the principles for building cellular ontologies published in this work allow deeper insights into the relations between the continuants (cell phenotypes) and the occurrents (cell mechanism changes) involved in cellular reprogramming, although implementation remains for future work. Further, our design principles lead to ontologies that allow the meaningful application of similarity searches in the spaces of cell phenotypes and of mechanisms, and, especially, of changes of mechanisms during cellular transitions. Richard Swinburne is one of the most distinguished philosophers of religion of our day. In this volume, many notable British and American philosophers unite to honor him and to discuss various topics to which he has contributed significantly. These include general topics in the philosophy of religion such as revelation, and faith and reason, and the specifically Christian doctrines of the Trinity, the Incarnation, and atonement. In the spirit of the movement which Swinburne spearheaded, the essays use analytic philosophical (...) methods to examine doctrines in particular religious traditions, expanding upon traditional discussions of theism. As such, this volume represents a field-report on the interaction of philosophy and Christian thought in the English-speaking world. Background Scientific and technological advancements in mapping and understanding the interrelated pathways through which biological and environmental exposures affect disease development create new possibilities for detecting disease risk factors. Early detection of such risk factors may help prevent disease onset or moderate the disease course, thereby decreasing associated disease burden, morbidity, and mortality. However, the ethical implications of screening for disease risk factors are unclear and the current literature provides a fragmented and case-by-case picture. Methods To identify key ethical considerations (...) arising from the early detection of disease risk factors, we performed a systematic scoping review. The Scopus, Embase, and Philosopher’s Index databases were searched for peer-reviewed, academic records, which were included if they were written in English or Dutch and concerned the ethics of (1) early detection of (2) disease risk factors for (3) disease caused by environmental factors or gene-environment interactions. All records were reviewed independently by at least two researchers. Results After screening 2034 titles and abstracts, and 112 full papers, 55 articles were included in the thematic synthesis of the results. We identified eight common ethical themes: (1) Reliability and uncertainty in early detection, (2) autonomy, (3) privacy, (4) beneficence and non-maleficence, (5) downstream burdens on others, (6) responsibility, (7) justice, and (8) medicalization and conceptual disruption. We identified several gaps in the literature, including a relative scarcity of research on ethical considerations associated with environmental preventive health interventions, a dearth of practical suggestions on how to address expressed concerns about overestimating health capacities, and a lack of insights into preventing undue attribution of health responsibility to individuals. Conclusions The ethical concerns arising with the early detection of risk factors are often interrelated and complex. Comprehensive ethical analyses are needed that are better embedded in normative frameworks and also assess and weigh the expected benefits of early risk factor detection. Such research is necessary for developing and implementing responsible and fair preventive health policies. (shrink)

Gipps IN THIS RESPONSE to Neil Pickering's paper I shall focus only on what he describes as the "strong objection" to the typical use of the likeness argument. The likeness argument, to recap, has it that we can decide whether conditions such as schizophrenia, depression, or alcoholism do or do not (...) deserve the designation of illness depending on whether they possess features which, if they occurred in physical conditions, would enable one to decide for or against the designation. To take an implausible example, if it were decided that it would be enough to describe a physical condition as an illness if it involves pain not caused through injury, then a psychological condition would be appropriately described as an illness just to the extent that it too involved such pain.In arguing against typical deployments of the likeness argument, Pickering finds that it is only on questionable initial descriptions of psychological conditions that such conditions appear to share the features that, in physical conditions, would be taken to be indicative of illness. That is to say, theorists who deploy the likeness argument to urge either the reality or unreality of mental illnesses typically presuppose what both we and they had all along hoped would be demonstrated in their argument. In terms of the above example, we might say that a certain psychological condition can count as an illness if it involves psychic pain. The concept of psychic pain is however (so the argument would have it) contestable; there exist other descriptions that make no reference to pain and in so doing fail to provide what a user of the likeness argument would need to substantiate the claim that a conception of the condition as an illness is more than a façon de parler.By way of arriving at this conclusion the following precondition for the applicability of the likeness argument (Pickering's "assumption" (2)) is insisted on: "That, with respect to the presence or absence of [those features of conditions which features decide whether or not the conditions are illnesses], a condition such as schizophrenia [must be] describable independently of the category it is assigned to." This can seem a quite reasonable restriction, for—it could be said—one can only win an argument if it is first allowed that there is actually something to argue about. That is, it is only if both partisans and detractors of the illness conception first accept a certain description of schizophrenia as spelling out what schizophrenia essentially involves that they can then progress to determining whether or not such a condition so described is or is not an illness.Yet, whereas this seems like a perfectly reasonable requirement, it is I believe uncertain whether it will always capture the form of the intuitions of adherents and detractors of the illness conception—or of the form of philosophical arguments more generally. Background: Despite procedural innovations and increasing numbers of uterus transplant attempts worldwide, the perspectives of uterus transplant (UTx) trial participants are lacking. Methods: We conducted a mixed-methods study with women with absolute uterine factor infertility (AUFI). Participants included women who had previously contacted the Cleveland Clinic regarding the Uterine Transplant Trial and met the initial eligibility criteria for participation. In-depth interviews were conducted in conjunction with FertiQoL, a validated and widely used tool to measure the impact of infertility on the (...) quality of life of infertility patients. Results: All (n = 19) rated their overall health as good; some experienced grief and social isolation. AUFI is a life-framing experience that influences acceptance by family, partners, peers, and one’s self. UTx is a means to gain control of reproductive autonomy. UTx allows family-building and the ability to play an active role in prenatal health and well-being. Establishing and maintaining a supportive relationship is a key issue of AUFI and when considering UTx. Risks of UTx are perceived relative to risks to self/child/family posed by adoption/surrogacy. Participants had no overall preference regarding living or deceased donor. Conclusions: The ways in which women with AUFI conceptualize this condition in their lives and choices around UTx and participating in a study of the procedure are multifaceted and textured. Mental and physical illness, deprivation and bereavement sometimes provide a propitious structure on which faith’s bright strands may grow. Scrutton posits no simply causal relationship between faith and mental illness, and steers us helpfully (...) away from the narcissistic overvaluation of inner experience too often met with in (1) mysterium tremendum (meeting-God-blew-my-mind), and (3) mental openness (divine-light-pours-through-the-cracks), formulations. She helps us to avoid settling, too, for the clever-sounding but ultimately too-easy idea of (2) ‘mental illness’ and ‘religious experience’ as different descriptions under which identical experiences may sometimes be brought. For one thing, and to animadvert now in my own vein: can we really—as William James aspires to, say, when he takes it for granted that the “more personal branch [of] feelings, acts, and experiences of individual men in their solitude, so far as they apprehend themselves to stand in relation to... the divine” can be neatly hived off from the “institutional branch of religion... worship... theology... ceremony... ” and so on—even individuate the relevant experiential phenomena independently of their meaning-conferring contexts? For another, it seems to me that too often in the literature of (1) (2) and (3) do we meet with a fatal confusion that can be expressed in terms of a conflation of two meanings of “ego,” as when a) the dropping of the kinds of boundaries which it makes sense to talk of putting up (i.e., our ego’s defensive, prideful, shame-driven shirking of intimacy) is muddled together with b) the crumbling of such individuating ego boundaries as constitute us. Such a confusion gets readily promulgated by vague and tacitly polysemic talk not only of ‘ego’ but also of ‘non-duality,’ ’transpersonal,’ and ‘higher’ and ‘lower’ ‘levels’ of ‘self’ and ‘consciousness’ (e.g., see Kasprow & Scotton 1999). Sometimes driving this, one can’t help suspect, is the narcissistic gain of conflating the dismal dissolution of ego boundaries in illness (Conrad, 1958) with the epiphanic transcendence of a grasping and fearful ego. Spiritual pride and spiritual bypass (i.e. fleeing the relational tasks of adult development into a self-preoccupied ‘spiritualised’ inner journey) (Welwood, 2000) then abound, not only for the sufferer but also for the lowly therapist who now receives their shamanic guide/hierophant upgrade. What particularly came to my mind was that notion of metanoia as offered us by the existential tradition in both theology and psychopathology.Mark’s gospel begins with John the Baptist heralding Christ’s arrival and preaching the “baptism of repentance [metanoia; μετανοίας] for the remission of sins” (Mark 1:4). Jesus’s first public words then proclaim the same message: “the kingdom of God is at hand: repent ye [μετανοεῖτε], and believe the gospel” (Mark 1:15). ‘Repentance’ is today often understood as a sorrowful acknowledgement of one’s wrongdoings, but it has long been acknowledged that, if that be the meaning we give the term, it is an inapt translation of ‘metanoia’ (Walden, 1896). In this paper, we empirically explore some manifestations of norms for the conduct of science. We focus on scientific research ethics and report survey results from 606 scientists who received funding in 1993 and 1994 from the Division of Molecular and Cellular Biology of the Biology Directorate of the National Science Foundation. We also report results for 91 administrators charged with overseeing research integrity at the scientists’ research institutions. Both groups of respondents were presented with a set of scenarios, designed (...) by fractional factorial methods, describing different kinds of scientific conduct that in the eyes of some would likely be unethical. Respondents then were asked to evaluate each of these scenarios for how unethical the behavior might be and what kinds of sanctions might be appropriate. We use the responses to consider the nature of consensus around norms related to the practice of science and in particular, similarities and differences between scientists and science administrators. The paper formulates and proves a strengthening of ‘Frege’s Theorem’, which states that axioms for second-order arithmetic are derivable in second-order logic from Hume’s Principle, which itself says that the number of Fs is the same as the number ofGs just in case the Fs and Gs are equinumerous. The improvement consists in restricting this claim to finite concepts, so that nothing is claimed about the circumstances under which infinite concepts have the same number. ‘Finite Hume’s Principle’ also suffices for (...) the derivation of axioms for arithmetic and, indeed, is equivalent to a version of them, in the presence of Frege’s definitions of the primitive expressions of the language of arithmetic. A brief, non-technical introduction to technical and philosophical aspects of Frege's philosophy of arithmetic. The exposition focuses on Frege's Theorem, which states that the axioms of arithmetic are provable, in second-order logic, from a single non-logical axiom, "Hume's Principle", which itself is: The number of Fs is the same as the number of Gs if, and only if, the Fs and Gs are in one-one correspondence.

Ethical decisions have to be made by everyone. Yet Christians often find it difficult to know exactly how they should make them. Is there a better way than an emotional response or looking for a quick answer from the Bible? In the first of the book a sixth-former, Matthew, takes moral issues as the topic of his RS project. He has interviews with various people, such as a businessman, a scientist, a police officer, and a doctor as well as with (...) members of his own family and his minister. Purpose This paper aims to explore the technologization of peace work through “remote support monitors” that use social and digital media technologies like social media to alert local violence prevention actors to potentially violent situations during demonstrations. Design/methodology/approach Using a distributed cognition lens, the authors explore the information processing of monitors within peace organizations. The authors adopt a qualitative thematic analysis methodology composed of interviews with monitors and documents from their shared communication and discussion channels. The authors’ analysis seeks to (...) highlight how information is transformed between social and technical actors through the process of monitoring. Findings The authors’ analysis identifies that the technologization of monitoring for violence prevention to assist nonviolent activists produces two principal and related forms of transformation: appropriation and hidden attributes. Monitors “appropriate” information from sources to fit new ends and modes of representation throughout the process of detection, verification and dissemination. The verification and dissemination processes likewise render latent supporting informational elements, hiding the aggregative nature of information flow in monitoring. The authors connect the ideas of appropriation and hidden attributes to broader discourses in surveillance and trust that challenge monitoring and its place in peace work going forward. Originality/value To the best of the authors’ knowledge, this study is the first to focus on the communicative and information processes of remote support monitors. The authors demonstrate that adoption of social and digital media information of incipient violence and response processes for its mitigation suggests both a social and technical precarity for the role of monitoring.
